Jerseyville nursing facility cited for code violations

JERSEYVILLE — The Illinois Department of Public Health (IDPH) revealed its nursing home violations report for the second quarter of 2024 on Wednesday, and a Jerseyville nursing facility is among the 268 violation reports completed.

Jerseyville Nursing & Rehab Center, located at 1001 S. State Street, was one of five facilities in the entire state cited with a type AA violation of the Nursing Home Care Act during this period, which is the most severe violation. A type AA violation is cited where there are conditions or occurrences at a facility that proximately caused the death of a resident.

The facility, owned by Springfield-based Helia Healthcare, was fined $50,000 for the type AA violation and a combined total of $27,200 for a type A violation and a type B violation.

IDPH performed the survey of the facility on March 11, in which the following three violations were reported:

  • Type B violation of an occurrence for violating one or more of the following sections of the Code: 300.610a), 300.1210b), 300.1210c), and 300.1210d)2)3)5). The fine was doubled in this instance in accordance with 300.282i) and j) of the Code due to the violation of the sections of the Code with a high-risk designation: 300.1210b) and 300.1210d)5). A fine of $2,200.00
  • Type A violation of an occurrence for violating one or more of the following sections of the Code: 300.610a), 300.1210b), 300.1210c), 300.1210d)6), and 300.1220b)3). The fine was doubled in this instance in accordance with 300.282i) and j) of the Code due to the violation of the sections of the Code with a high-risk designation: 300.1210b) and 300.1210d)6). A fine of $25,000.00
  • Type AA violation of an occurrence for violating one or more of the following sections of the Code: 300.610a), 300.696a), 300.696b), 300.696d)6)13)16), 300.696f)2)A)4)7), 300.1210b), and 300.3240a). The fine was doubled in this instance in accordance with 300.282i) and j) of the Code due to the violation of the sections of the Code with a high-risk designation: 300.696, 300.1210b), and 300.3240a). A fine of $50,000.00

Requests by Heartland Newsfeed to Helix Healthcare were not returned as of Wednesday evening.
